Arise is proud to offer our patients Mobile Treatment Services. This highly effective program, based on the Assertive Community Treatment (ACT) model, provides comprehensive and individualized mental health care to individuals with serious and persistent mental illness in the community setting.
The program is designed to provide wrap-around services that address all aspects of an individual’s life, including housing, employment, substance use and social support. Our team of highly trained and licensed social workers, therapists, nurses and support staff work together to provide a personalized treatment plan that meets each client’s unique needs and goals.
Individuals who meet the following criteria may be eligible for the program:
- Diagnosis of serious and persistent mental illness: Individuals must have a diagnosed mental health condition that significantly impacts their daily functioning and quality of life.
- Need for intensive support: Individuals must require a high level of support and care in order to manage their mental health condition and live in the community. This can be indicated by frequent arrests, emergency room visits, and hospitalizations related to mental health.
- Difficulty accessing traditional mental health services: Individuals who have a history of difficulty accessing and participating in traditional in-office mental health services.
- Residence in the community: Individuals must reside in the community and may be homeless, rather than in a hospital or other institutional setting.
